Do you know the consequences of incorrect prescription or administration?<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-3dbc493c40c111179ebcf3bba2b2d8c0 wp-block-paragraph\">At some point in our lives, we've all used medication to treat an illness. Although these medications undergo rigorous and extensive processes to ensure their safety, there are still some medication and dispensing errors that can harm patients.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-e4a43fdede2e870815b42e5eb303e4be wp-block-paragraph\">Every year, a large number of patients suffer harm or die as a result of unsafe healthcare, causing a high burden of death and disability worldwide, according to the World Health Organization (WHO).\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-a46a6df85a703d28b6aca7799a712883 wp-block-paragraph\">Keep reading to learn about these errors, their possible causes, and how to prevent them to improve the safety of medical treatments in Latin America.\u00a0<\/p>\n\n\n\n<h5 class=\"wp-block-heading\"><strong>What Are They?<\/strong><\/h5>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-45a6df204e3752623f5da95cad74251f wp-block-paragraph\">According to the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP), medication errors (MEs) are defined as any preventable event that may cause harm to the patient and is related to the inappropriate use of medications; while the medication is under the control of the healthcare professional, the patient, or the consumer.<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-10495cef73b37d8e055e9189b58cbddb wp-block-paragraph\">Some of the most common errors include <strong>prescripci\u00f3n<\/strong>, <strong>incorrect prescription, preparation, dispensing, distribution, or dosage; and incorrect administration and consumption <\/strong>of medications.\u00a0<\/p>\n\n\n\n<h5 class=\"wp-block-heading\"><strong>Causes and Consequences<\/strong><\/h5>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-402ddd24203d3e5f853015cadae11584 wp-block-paragraph\">Now that you know the most common errors, imagine this scenario:<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-2673b989d2a69ed58c3baa18c29b309f wp-block-paragraph\">A patient has just been admitted to the hospital due to an urgent medical condition. The doctor prescribes a vital medication for the patient, but due to the urgency of the situation, the information about the exact dosage and administration time is unclear. In the dispensing area, the pharmacist interprets the prescription differently and prepares a dose that does not match the doctor's intention. Then, during the medication administration, the nurse administers the wrong dose.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-3181004a19bc68db5e2dba2d867ccd8f wp-block-paragraph\">This example can happen for various reasons. Here are some of the most common causes:<\/p>\n\n\n\n<ul class=\"has-black-color has-text-color has-link-color wp-block-list wp-elements-30a300c740fed04cfa1f565558a39ce6\">\n<li class=\"wp-elements-001dbbf0d539681219eb9a4d52ac0e22\"><strong>Ineffective communication: <\/strong>Information is not conveyed clearly between healthcare staff and the pharmacist, and incorrect or incomplete information may even be provided to patients. Ineffective communication also involves language barriers.<\/li>\n\n\n\n<li class=\"wp-elements-87d7581fcfbcdc657aea4a3028b75374\"><strong>Lack of training: <\/strong>Some healthcare professionals do not have the necessary training to correctly address medication administration.<\/li>\n\n\n\n<li class=\"wp-elements-9aad8cf8747327f5bbc3fea7e68bc7d9\"><strong>Challenging healthcare systems:<\/strong> The healthcare infrastructure in some countries in the region may pose challenges such as limited resources, lack or excess of staff, and long working hours leading to tired healthcare professionals, increasing the likelihood of errors.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-8f0a3b29403e28b8dc54943ee3149211 wp-block-paragraph\">Medication errors have a detrimental impact on people's lives, causing patients to become ill or worsen their condition. Some of the consequences include:\u00a0\u00a0<\/p>\n\n\n\n<ul class=\"has-black-color has-text-color has-link-color wp-block-list wp-elements-25d9ba3c68d2bccd9c06b09e077751ec\">\n<li><strong>Health risk:<\/strong> Consuming the wrong medication or an incorrect dosage can lead to hospitalization, disability, birth defects, and even death.\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"has-black-color has-text-color has-link-color wp-block-list wp-elements-fbc3528c7562f210df975359f7ad2f99\">\n<li><strong>Distrust in the system:<\/strong> Repeated errors can undermine trust in the healthcare system, making people hesitant to seek treatment when needed.<\/li>\n<\/ul>\n\n\n\n<ul class=\"has-black-color has-text-color has-link-color wp-block-list wp-elements-bcd529124e834e1629c69238ca405152\">\n<li><strong>Economic burden:<\/strong>Additional treatments and unnecessary hospitalizations can generate significant costs.\u00a0<\/li>\n<\/ul>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-36b4c3a8833e9913eca7fc8a440a1ea5 wp-block-paragraph\"><em>Seg\u00fan la Organizaci\u00f3n Mundial de la Salud (OMS),\u00a0 el costo social de los da\u00f1os a los pacientes puede valorarse entre 1 y 2 billones de d\u00f3lares al a\u00f1o.\u00a0<\/em><\/p>\n\n\n\n<h5 class=\"wp-block-heading\"><strong>Overview in Latin America<\/strong><\/h5>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-e6b69570a0d78b9ebe2253b59c1b56ee wp-block-paragraph\">Health is invaluable, and medical care is essential to ensure the well-being of the population. However, despite efforts to provide quality medical care, Latin America faces significant challenges related to medication and dispensing errors. Research conducted in the region reveals higher rates of adverse events than those identified in developed countries.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-3aabdab0bddd5474ed2e7d0bdd34a2fb wp-block-paragraph\"><em>Available evidence indicates that hospitals in low- and middle-income countries experience 134 million adverse events and 2.6 million deaths annually (WHO, 2023).\u00a0<\/em><\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-70784e0019bcf70d621e3448381990b2 wp-block-paragraph\"><strong>Estas cifras, superiores a las de los pa\u00edses desarrollados, podr\u00edan atribuirse a la menor calificaci\u00f3n de los profesionales de la salud y a la infraestructura inadecuada de los sistemas de salud. La literatura disponible sugiere que de 20 pa\u00edses de latinoam\u00e9rica analizados, los errores m\u00e1s\u00a0 comunes fueron los de prescripci\u00f3n y administraci\u00f3n, as\u00ed como los errores humanos asociados a la atenci\u00f3n en salud.\u00a0<\/strong><\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-04f3a5dbca6b182338ac178f55c197bc wp-block-paragraph\">Specifically, the most common administration errors are related to timing, dosage, and omission. The former occurs when patients take medication at the wrong time, the second when the dosage is inaccurate or not prescribed, and the latter when the patient does not take the medication.\u00a0\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-fb2fbd6ccd91db1cde216cbcfa42b909 wp-block-paragraph\">However, the literature reveals a lack of standardized classification in the definitions of MEs among Latin American countries, highlighting the need to establish uniform criteria to assess and address this critical issue. For example, eight countries do not consider the definition of medication error (ME) within their official documents. Among these are Panama, Colombia, the Dominican Republic, and Venezuela.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-25e8fb50feda6a92ccdca3206b2d975e wp-block-paragraph\">Medication and dispensing errors represent a critical challenge to patient safety in Latin America. Despite advances in healthcare, error rates remain significant, with potentially serious consequences for the health and well-being of the population.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\"><strong>Steps towards a Safer Future<\/strong><\/h5>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-442f44a69e5679b199140b11fc258cc8 wp-block-paragraph\">How do we effectively address this problem? First and foremost, it is essential to promote ongoing research, establish regional standards, and encourage collaboration among healthcare professionals, policymakers, and healthcare institutions.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-7172d7f30edd377802a82308c64af2dd wp-block-paragraph\">The implementation of advanced technologies, improvement of healthcare staff training, and public awareness are also crucial steps towards a safer future in medication administration in Latin America.<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-aa8359457a13d748929c64a8478a6dd8 wp-block-paragraph\">A valuable resource is the <a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240032705\">Global Patient Safety Action Plan 2021-2030<\/a>, led by the World Health Organization (WHO), a strategic direction for concrete measures to be adopted by countries, partner organizations, and healthcare establishments; to eliminate preventable harm in healthcare and ensure patient safety.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-db4c1ea5060bd4b90f450b5bada5f705 wp-block-paragraph\">In our region, it is important to heed the WHO's call to join the mission of driving policies, strategies, and actions; to eliminate all sources of preventable risk and harm.\u00a0<\/p>\n\n\n\n<p class=\"has-black-color has-text-color has-link-color wp-elements-d9645ca861baa549282b56dbbb407a81 wp-block-paragraph\">Now that we understand the causes and consequences of medication errors (MEs), and have reviewed the landscape in the region; we are one step closer to building a stronger and more reliable healthcare system for all.
